For high school students graduating in 2009.

Applications must be turned in to your school counselor on or before April 15, 2009. You must be a 2009 high school graduate from Yuma or La Paz county.

1. Please complete the application, sign and date.
2. Submit a high school transcript.
3. Attach three letters of recommendation.
4. Attach a brief description of your school and community-based honors, awards, and activities.
5. Write a personal statement describing your educational and career goals, and an explanation of why you want or need a scholarship.
6. Once completed, please print and turn in to your high school counselor.

A current copy of high school transcript(s) must be submitted with this application.
